ABOUT THE PROJECT
Goals & Objectives
- Goal 1:
Youth with special health care needs and their families will collaborate in decision making at all levels and will be satisfied with the services they receive. - Goal 2:
All children and youth with special health care needs will receive coordinated, ongoing, comprehensive care within a medical home. - Goal 3:
All children and youth will be screened early and continuously for special health care needs. - Goal 4:
All youth with special health care needs and their families will have sufficient private and/or public insurance to pay for the services they need. - Goal 5:
Services for children and youth with special health care needs and their families will be organized in ways that families and youth can navigate and use them easily. - Goal 6:
All youth with special health care needs will receive the services necessary to make appropriate transitions to all aspects of adult life, including adult health care, work, and independence.
Project Activities
Community development and systems integration will include the following activities
- Expanding access to medical homes
- Introducing and expanding care coordination
- Assuring access to adequate and coordinated services
- Providing youth leadership in self determination
Strengthening the infrastructure of the CSHN program will include the following activities
- Implementing the Cast-5 assessment findings
- Establishing and supporting the six regional family and youth advisory committees.
- Coordinating with the regional family to family Health Information Centers
- Enhancing the impact of families and youth to influence policy and practice
- Enhancing lingual and cultural competency
