Overall Purpose

Integrated Community Systems for Children and Youth with Special Health Care Needs in Maine is designed to assure that Maine’s children and youth with special health care needs (CYSHN) and their families have access to an integrated, community based system of care achieving the goals of the Maternal and Child Health Bureau’s six National Performance Measures.

This grant is funded by the U.S. Department of Health & Human Services, Health Resources and Services Administration, Maternal and Child Health Bureau (award #D70MC06895). The grant was awarded to the Children with Special Health Care Needs program at the Maine Department of Health and Human Services’ Center for Disease Control and Prevention. Project partners include the Maine Support Network, Maine Family Voices and the Maine Chapter of the American Academy of Pediatrics.
